Excessive Daytime Sleepiness: How Doctors Diagnose and Treat It

When excessive daytime sleepiness continues despite enough time in bed, a systematic evaluation is important. The goal is not merely to increase alertness; it is to identify and treat the reason restorative sleep or normal wakefulness is being disrupted.

The first consultation

Your clinician may ask when symptoms began, how often you doze, whether naps are refreshing, how difficult it is to wake, and whether sleepiness affects driving or work. Questions about snoring, breathing pauses, leg sensations, dream-like experiences, sleep paralysis and sudden muscle weakness help narrow the possibilities.

Reviewing sleep opportunity and routine

Many adults unintentionally sleep too little. A sleep diary records bedtime, estimated sleep time, awakenings, wake time and naps. Actigraphy, a wrist-worn movement monitor, may be used to estimate sleep-wake patterns over several days. Shift work, travel and irregular weekends are also considered.

Checking health and medicines

Clinicians review medical conditions, mood symptoms and all prescription, non-prescription and herbal products. Some medicines can cause drowsiness or disturb sleep. Blood tests may be appropriate when anaemia, thyroid disease, iron deficiency or another medical contributor is suspected.

Sleep testing

  • Polysomnography: Records brain activity, breathing, oxygen, heart rhythm and movements overnight.
  • Home sleep apnoea test: Used for selected adults when uncomplicated obstructive sleep apnoea is suspected.
  • Multiple Sleep Latency Test: Measures how quickly a person falls asleep during scheduled daytime nap opportunities and looks for early REM sleep.

The Multiple Sleep Latency Test is usually interpreted with the preceding overnight study and an adequate, documented sleep schedule. It should not be treated as a stand-alone online โ€œsleepiness test.โ€

Treatment depends on the diagnosis

Sleep restriction calls for protected sleep time. Sleep apnoea may require positive airway pressure, an oral appliance or another selected therapy. Circadian disorders need correctly timed schedule and light strategies. Narcolepsy and idiopathic hypersomnia require specialist care that may include prescribed wake-promoting treatment and planned naps.

Do not ignore dangerous drowsiness

Avoid driving or hazardous work when you cannot stay alert. Seek prompt medical advice if sleep attacks are sudden, frequent or associated with weakness, confusion or breathing problems.
